Nica Pierce has been locked up in an asylum for criminally insane people for four years. She is wrongly convinced that she, not Chucky, killed her whole family. But when Nica's psychiatrist brings in a "therapeutic tool"—a "Good Guy" doll with a smiling face—to help with the group sessions, a series of horrible deaths start to happen in the asylum, and Nica starts to wonder if maybe she isn't crazy after all. At the same time, Chucky's enemy from the first three Child's Play movies, Andy Barclay, who is now an adult, runs to help Nica. But in order to save her, he will have to get past Tiffany, who used to be Chucky's wife. She will do anything, no matter how dangerous or bad, to help her evil devil doll.
